Who can use this program?
Children ages 0-18: For information on how to enroll a child as a dental patient, please read on.
Adults: We are currently at capacity for new adult patients and cannot process applications for adults at this time. Please check this page for updates after April 1, 2021.
To access emergency dental services, you must be an established health center patient and/or an established dental patient.
What should I do first?
Please download, print, fill out, and return our intake paperwork, as explained below. If you don’t have access to a printer, please call us and ask for the packet to be sent to you by mail.
The information below is for parents who want to enroll children as dental patients. Please do not send applications for adults at this time.
- Current patients at Families First who are under age 18 and have never used dental services before should use this intake packet.
- Patients who are new to Families First and are under age 18 should use this intake packet.
- Email the paperwork to us using this link, or mail or bring it to: Dental Department, Families First, 100 Campus Drive, Portsmouth, NH 03801

Once we’ve processed your paperwork, we will call you to schedule your first appointment. When scheduling, let us know if you will need translation.
Arrive 10 minutes early to finalize paperwork. Bring your forms if you haven’t mailed them in. Ask for a Patient Handbook.
Bring a list of the medications you take, or the actual medications.
Bring your insurance copayment or sliding-fee scale payment.
