Diabetes Group Visits

For patients of Families First Health Center who have diabetes, Families First offers a group-visit program in addition to providing one-on-one care. This innovative program has been featured on New England Cable News and NH Public Radio, and we've found that patients who participate in the groups have improved control of their diabetes.

The group visits include:

  • Diabetes health screenings
  • A short private visit with a nurse practitioner
  • A healthy snack
  • A chance to exchange tips and experiences with others who have diabetes
  • An educational presentation about an aspect of managing diabetes, such as exercise, diet or mood.

2012 Group Visit Dates:
January 20, February 24, March 16, April 20, May 18, June 15
Fridays, 10 a.m. to noon
RSVP by the Wednesday before each meeting: 603-422-8208 ext. 222
