Families First is accepting new patients in primary, dental, pediatric and prenatal care. We’re open to everyone – insured or uninsured, infants to seniors, individuals and families. All are welcome at our parenting & family programs too. Why Families First? Because we treat the whole person, and we make it easy!

What should I do first?
  • If you have a printer, you may fill out this Patient Intake Form (.pdf). Or you may ask us to mail you one.
  • Read these pages from our Patient Handbook (.pdf)
  • Print and sign the form.

Then what?

If you have insurance, call to 603-422-8208 (press 1) to schedule your appointment. If you don’t have insurance, once we have processed your sliding scale application, we will call you to schedule your first appointment. When scheduling, let us know if you will need child care or translation.

Arrive 5-10 minutes early to finalize paperwork. Bring your forms if you haven’t mailed them in. Ask for a Patient Handbook.

Bring a list of the medications you take, or the actual medications.

Bring your insurance co-payment or sliding-fee-scale payment.

    If you have questions, call our prenatal nurse at 603-422-8208 ext. 108, or our social worker at ext. 142.
Then what?

Our prenatal social worker will call you to schedule your appointment. Let her know if you will need child care or translation.

What should I do first?

Fill out the Sliding Fee Application (.pdf). (This is also the last page of our Intake Form, so if you recently filled out that form, you’re all set.) If you have questions, call 603-422-8208 ext. 1

Then what?

After we receive your paperwork, we’ll call you to schedule your first appointment and let you know how much it will cost. Let us know if you will need child care or translation during your appointment.

Who can use this program?

Anyone who is at Families First for an appointment or family program

What should I do first?
  • If you need child care during a Families First appointment (health, dental, or insurance enrollment), ask the receptionist to schedule you for a time when child care is available. Then reserve your child’s spot online or at 603-422-8208 (press 2).
  • If you need child care during a parenting class or group, sign up for program and child care online or at 603-422-8208 (press 2).
  • Fill out the Family Center Registration Form (.pdf) now, or when you come to your first program.
Then what?

Come to the program or appointment you signed up for. Arrive a few minutes early to get your kids settled in child care (first door to the right of Families First entrance).

Our Family Services Director will talk with you about your needs. If she thinks a home visitor could help, she will have someone get in touch with you to get started.
